First, the state must create, expand, and sustain a dementia-capable HCBS system that includes Single Entry Point/No Wrong Door (SEP/NWD) access so that people living with dementia and their caregivers can find and obtain services without being bounced between multiple agencies or confusing eligibility channels. Second, the state must ensure access to a comprehensive and sustainable set of high-quality, dementia-capable HCBS, including innovative services that better meet the needs of people with dementia and those caring for them. Eligibility for Option A is limited to states and territories that do not currently have an active ACL ADPI State dementia-capability grant. While any state without an active grant may apply, priority in final funding decisions is given to states that have not benefited from Alzheimer’s Disease Supportive Services Program (ADSSP) grants since before 2014, reflecting an intent to direct resources to places that have had fewer recent opportunities to build this infrastructure.
Option B, Grants to Communities, targets public or private community-based organizations (CBOs) that are already operating within an existing dementia-capable HCBS system and can clearly explain how additional services will strengthen that system by filling specific, well-defined gaps. Rather than building an entire statewide system, Option B is designed to help local providers deepen and improve community-level supports for people living with or at high risk for ADRD and their caregivers. Under Option B, applicants must address all three required gap areas. The first is providing effective supportive services for people with ADRD who live alone in the community, a group that often faces elevated risks related to safety, isolation, unmet daily living needs, and limited informal support. The second gap area is improving the quality and effectiveness of programs and services for individuals aging with intellectual and developmental disabilities (I/DD) who have ADRD or are at high risk of developing it, recognizing that this population may need approaches that are tailored to lifelong disability supports and different caregiving environments. The third required gap area is delivering behavioral symptom management training and expert consultations for family caregivers, which focuses on practical help for managing dementia-related behaviors and reducing caregiver stress, burnout, and avoidable crises that can lead to emergency care or institutional placement. As with the state option, CBOs may only hold one ADPI grant at a time, and priority consideration after review is given to organizations that have not benefited from ADI-SSS or ADPI grant programs since before 2014.
