The Public Education for Living Organ Donation Reimbursement Program (PE-LODRP) is a FY2025 Health Resources and Services Administration (HRSA) cooperative agreement designed to increase awareness and use of HRSA's Living Organ Donation Reimbursement Program (LODRP), especially in communities that have historically been underserved by the program. The opportunity exists because the United States continues to face a major shortage of transplantable organs, particularly kidneys and livers. As of March 2024, about 89,000 people were waiting for a kidney and about 9,800 were waiting for a liver, with those two organs representing roughly 96 percent of the total need on the national transplant wait list. While living donation can help address this gap (for example, a person can donate a kidney or part of a liver), living donation numbers remain far below the level needed; in 2023, nearly 7,000 people donated an organ while alive, including around 6,300 kidney donors and 660 liver donors.
LODRP is the existing HRSA program that PE-LODRP is meant to promote and expand. LODRP reduces financial barriers that can prevent someone from becoming a living donor by reimbursing qualified expenses tied to the donation process, including costs related to medical evaluations, surgery, and follow-up care. Reimbursable categories include travel and other subsistence-related costs, lost wages, child care, elder care, and additional eligible expenses described in HRSA guidance on reimbursement for living organ donation. The program has a strong track record: from October 2007 through August 2024, it received more than 18,000 reimbursement applications from prospective living donors and approved nearly 89 percent of them, helping facilitate over 10,000 living organ donations during that period. The financial-need rationale is clear in the applicant profile as well; in 2023, the median household income for prospective living donor applicants was about 66,000, roughly 18 percent below the national median of 80,600.
A core driver for creating PE-LODRP is equity in access and outcomes. Even though more than 60 percent of people on the national transplant waiting list are from a minority group, only about 40 percent of patients from a minority group received a living organ transplant through the program. HRSA is using this new public education and outreach award to expand awareness of LODRP and to improve access to reimbursement support for medically underserved communities, with the goal of reducing financial and informational barriers that disproportionately affect certain populations. Under this cooperative agreement structure, the award recipient is expected to work closely with HRSA and with the separate LODRP award recipient (currently operated by the National Living Donor Assistance Center) to coordinate messaging, outreach strategies, and education efforts that drive eligible prospective donors and recipients toward the reimbursement resources.
